DESCRIPTION:\n\n\n\n\nKOC has set the date and times for their school physicals for Saturday, June 13th.  Gibbs time slot is at 1:30pm.\nThis year it will be at their Weisgarber Office at 1422 Old Weisgarber Road, Knoxville.  (NOT Thompson Boling Arena)\nCost is $10 per athlete.  Cash or Check only (No debit or credit cards)\nMasks are required\nPrint and complete pages 1, 2 and 5 of the KCS Physical Form for Athletes PRIOR to the exam. The form can be found at https://www.knoxschools.org/Page/7295\nCOVID Release form to complete when you arrive\n\n\n\n\nFrom KOC:\n\nDue to the ongoing coronavirus pandemic, and in the best interest of public safety, several precautions will be taken as outlined below:\n\n\nIf you have recently been exposed to the coronavirus, or you have tested positive – PLEASE DO NOT COME.\nLocation will be the KOC Weisgarber Office – 1422 Old Weisgarber Rd., Knoxville TN. (NOT Thompson Boling Arena)\nPhysicals will NOT run as they have in the past.  Instead, they will be operated like a typical physician visit. Check in > pay fees > placed in an exam room to be seen by practitioners to complete the physical.\nCost will be $10 cash or check.  No credit / debit cards.\nPages one, two and six of the physical form MUST be completely filled out prior to the physical.  This includes all applicable parent signatures.\nThe waiver form MUST be completed and signed prior to the physical.\nEveryone entering the building is required to wear a mask. No mask, no physical.  KOC will NOT be providing masks.\nAll persons will be screened prior to entering the building.\nAs space allows, there will be a limited number of people allowed in the lobby at any given time. But please understand that you may be asked to wait outside / in your car and will receive a call/text when it’s your turn.\nNo one will be allowed to come back to the room with the athlete. If there are questions, we will call the parents. This is to limit exposures.\nAfter the physical, the athlete will exit the building. There will be no loitering permitted,\nThere will be frequent cleaning of the lobby chairs.\nPatient rooms will be cleaned after every athlete.\nAdditional safeguards may be added. This is a brief summary and not an exhaustive list of the measures being taken to ensure the safety of our staff, patients and student athletes.\n\n\n\n\n\n\n\n
